PREFACE TO THE FOURTH EDITION
THE THERAPEUTIC CHOICES INCREASE
The dilemma of therapeutic choices was discussed in the preface of the third edition of Current Therapy in Cardiovascular Disease. As explained in that preface, new drugs and new procedures for the treatment of cardiovascular disease become available every few months. In addition, certain treatments, previously believed to be useful, are abandoned after they have been used for a few months or years in the field. Because of these changes, it seemed desirable to shorten the interval between the editions of this book from four years to three years.
In order to update the book after only three years, it seemed wise to ask the same authors who contributed to the third edition to contribute to this edition of the book. As editor, I was grateful that all of the authors who contributed to the third edition agreed to update their contribution (if needed) for the new fourth edition of the book. Most of the chapters required additions and deletions. Some chapters required major revisions, while others required only a few changes. A few chapters required no changes.
The successful format has been continued in this edition because it assists the reader in making therapeutic choices. Furthermore, in recognition of the time constraints placed on practitioners, it is my hope that their questions about treatment can be answered quickly when they refer to this book.
