Preface to the Second Edition
The acceptance of the first edition of our book has been very rewarding. Encouraged by this favorable response, we have now prepared the second edition, made necessary by the significant advances in diagnostic cardiology during the last five years. Progress has been most spectacular in the area of noninvasive techniques (Chapter 12), but sizable gains have also been registered in the field of myocardial mechanics (Chapter 10). These two sections have been completely rewritten and expanded greatly. A new chapter on the diagnosis of coronary artery disease (Chapter 11) has been added to which Drs. V. Maranhao and H. Goldberg have made a valuable contribution.
The addition of a thorough discussion of catheter-transducer frequency response has remedied an omission of the first edition. Chapter 2 has been expanded by a detailed description of the theoretical and practical aspects of Pick cardiac output determination. The development of new technics and formulae to measure the volume of all cardiac chambers and the increasing availability of pertinent normal data is reflected in the extensive updating and reorganizing of Chapter 5. All the other chapters have been revised and expanded where appropriate.
A number of illustrations have been added, some new and some replacing and improving upon ones in the previous edition. The material for the new illustrations comes from the Hemodynamics Laboratories of Deborah Heart and Lung Center, Browns Mills, New Jersey, and the Medical College of Pennsylvania, Philadelphia, Pennsylvania. The bibliography has also been updated.
